I have seen a bible used in NY, but not often (although I have seen many bibles in the courtrooms).

In addition to the oath you set forth I have had a few witnesses who affirmed rather than swear, but you don't usually notice it.

I was in a small town court a couple of months ago and watched a Judge who had no clue attempt to perform a preliminary hearing. The Prosecutor asked the Judge to swear in the witness and the Judge looked said "I forgot how to do that"
