According to The Gaming Table, there was a Dukes Smoke Shop in Paso Robles, CA that used a LargeCrown chip. This place is shown as closed.

There is also a Dukes Card Club in Fresno that is still in operation. No chip mold is indicated.

The Gaming Table has the same information you have on the Cahuilla chip. I don't know where Anza, CA is either.
