I have a Blue Book - First Edition # 124, purchased directly from Borland.
Mine includes tabs 6 thru 10, although these were just places to file your sales lists.

I haven't compared recently, but the pics and numbering system for chip molds is similar (or identical?) to Howard Herz' publications.

I don't think there was ever a 2nd edition. Just the empty tabs that you could put your 3-hole punched sales lists & some other stuff he would send.
