NCR, Shipping damage sad

Can anyone explain to me how a package containing a fairly sturdy, thick glass dish can arrive with the dish shattered and broken and the box totally undamaged.

Do you think it was already broken when it was packed or could this happen in shipment and the box not show any sign of damage?

The box is a typical brown carton. The dish was wrapped in bubble paper and crumpled newspaper used to fill the box.

Any ideas?

Was the package marked as being "FRAGILE"?
