I bought this chip and license on ebay about a year ago from William Bowman who lives in Carson City. He said that he got it from his grandfather Len Haffey Sr. (who is listed on the license) and that it came from the Delta Club. Here's a pic of the chip and if you click the link you can see a pic of his grandfather. I've traded the chip and license away since then.
