The "M" logo is from the M Resort in Las Vegas (south of South Point).

But I don't think they would have used the chip in any tournament or promotion, even as a No Cash Value. My guess is that it's a gift shop / souvenir or similar chip, as made on a non-casino quality mold and without the minimum identifiers that NGCB requires.
