Could've been a mis stamp where it was pressed twice in the mould, it looks more like the chip was warped and someone tried to flatten it, and pressed another horse head chip into it, leaving additional heads in the face. I have a 36mm chip that has this with hat/canes, I am unsure if it is a double press from Paulson, or if the previous owner tried to flatten it and pressed additional hats into the chip face
