First row, 4th chip, is listed as SqSqDi. You may have missed it because it is often described in other sources as DiSqSq.

Third row, 3rd chip, is listed as 16Suits (4C). It was not made by Caro, the similar Caro mold is called 16Suits (Caro), and is listed below it.

Third row, 4th chip, is listed as LgSqur.

Chips on the same mold as the one in Row 2, second chip, show up when you query DiSq. The 2nd row, 3rd chip shows up when you query Horshu, Row 3, 2nd chip, comes up when you query Diamond. Listing these variations you have in the Chip Mold Guide might be useful.
