Here are the regular $1 chip, the $1 error chip with the $5 inlay on one side and the regular $5 dollar chip. I always assumed that inlays were cut and printed and then applied to the clay. So it would have made more sense that somehow the wrong inlay was used. But in this case it seems the wrong denomination was put on the correct inlay shape.
