Nevada Gaming Tokens, that issue was from 1997. There is also a 1997 issue for a $1 (CT) and a $2 (also GDC like the $5) and all 3 are attributed to South West Gaming as the "Route Company". There was an earlier (1995) $1 issue by CT, with E-T-T, Inc. as the Route Company.
