I have been looking for this chip for almost 2 years without any success.
This book is as up to date as you can get. It is the culmination of a lot of work for a few people. Mainly Robert "Smitty" Smith, Mick Cuddy (this is on his web site), Roy Klein and a couple of others who helped supply some chip scans.
