In his outstanding book "The Rise of the Biggest Little City", Kling lists the occupants of 222 Lake Street as follows:

1956 - Lido Club
1956-1957 - Old Cathay Club
1957 - California Club (Never Licensed)
1958-1960 - Happy Budda
1962-1964 - Lido Club (again)

While it was the Old Cathay Club the NGCB revoked the owner's (Harry Chon) license for "using improper methods of operation". I suppose that could have included using unapproved chips but there are no hard cold facts that I could find to help verify this id.
