That matches the one in Ralph's and Ginsburg's book. Mustard with Black. Both list it from the Curacao Hilton.

Interesting someone else thinks it is from Puerto Rico.

Based on the fact I know the $5 matching chips came from someone while visiting Curacao, I would have to guess it is a Curacao chip. Unless it was used in both places.
