Don--the smallest I could find was the Avery 8167 return address label which is 0.5" by 1.75". If you have your collection in a spreadsheet or database you can use the mail merge to generate and print all of the labels in a batch. Attached is a sample--the label is wrapped around the bottom of the 2 1/2 by 2 1/2 holder to seal it shut as well. Fitting your information onto one of these labels can be a challenge, mostly trial and error, in part because the font sizes are limited. If you have Microsoft Publisher you get a little more flexibility with spacing, etc.
