Unless I missed it, James' third-edition book doesn't list this chip, which I've always attributed to the Cal-Vada Lodge on the north shore of Lake Tahoe. The Chip Rack lists the chip (N0750) but with an "attributed to" designation.

Does anyone have any documentation as to where this chip was used? If not Tahoe, then where? Thanks in advance for your help.
