With a blank reverse and two passes through the hot-stamp machine, I'd say it's more likely a "setup chip" used to get the heat and impression depth right before producing quantities. Some call them "floor sweepings", as they were likely never delivered to a customer. I think it's reall stretching to call these examples errors, Richard, but of course that doesn't keep anyone from collecting them.
