CC&GTCC Hall of Fame Recommendation Form. I am fairly certain that an electronic version can be had from Archie Black, Chairman Hall of Fame Committee.

The other insert was the 2004 Bruce Landau Memorial Auction submission form (the same one that was sent out (prematurely) last issue. However, if you need one, I image the electronic version might be available from Barry Weintraub, Auction Chairman.

I also suspect that Allan Anderson might be able to send electronic versions of both, or even put up PDF versions (if he reads this).

Lastly, my copy came with a third insert; Salnicky's copy of the magazine!
