I found another one a few weeks ago. On P.135 in TCR-9, look at the Harrah's dimes. They are sorted:
HUB
SM-KEY
SM-KEY
8-SUITS
SM-KEY

I asked Ernie Wheelden. He said that a character sometimes get in there without actually showing up. To make the correction, Ernie had me erase the mold and retype it. He said that it will sort properly the next time. I will retype the Fremont dimes, also. Thanks
