Also, you should prepare a notarized statement describing the circumstances under which the repairs were accomplished, in triplicate, one copy of which should be submitted to the Club Secretary for maintenance in the soon-to-created CC>CC "Repaired, restored, altered, modified and otherwise non-genuine casino collectible" file, one copy maintained by you and your heirs for a minimum term of your life plus 21 years and one copy of which should be maintained at all times in the immediate vicinity of the item.
