A misunderstanding doesn't automatically make a chip issued an error chip. A design flaw can call for a casino to remove a chip from play, be it voluntary or involuntary. An error chip is produced at the point of manufacture when a chip is made that differs from the rest of the chips nade during the same manufacturing process.

Using the word "error" to describe chips in any other manner dilutes the meaning of the word when used to describe real error chips.
