Here's a set of 3 Caesars Palace $5 chips (all House mold) -- top row shows obverse; bottom row shows reverse:

The first chip is the normal house-mold chip with 3 sets of inserts. The second chip is the "error" chip with 4 sets of inserts (this one already listed in TCR).

The third chip is a "double error" chip, in that it has the 4 sets of inserts, and ALSO, the reverse side has a $25 inlay placed on it. This $5/$25 "double error" chip is not in TCR-8.
