Yes that helps.
I have kind of "cheated" when mailing to Scott - keep the number of chips low and used a legal sized envelope and a lot of tape! No form.

I have to mail a few more than usual and hoped I could get away with a straight mailing - no forms - no hassle. I am sure I can.
