As far as your PAC chip goes:
The note saying "O'Neal in 1927" is probably meant to say E.M. O'Neil Co.
They were a Chicago gaming suppy Co. My guess is the chip is in the USPC records as being shipped to O'Neil for resale. If this is what was meant by the note, then the trail will probably stop here as there are no records from the O'Neil Co. They moved to Las Vegas and distributed the Cord mold chips for quite some time. I talked to O'Neil's daughter, she ran the Co at the end. She says they destroyed all records of the Co.
Maybe John B or Allan will jump in here and be able to confirm my assumptions about USPC.
