My first impression of this chip may be the correct answer. I bought one lot of miscellaneous chips on eBay two years ago and they included what I call "floor sweepings". These are the mistakes that are thrown on the floor to be thrown away. They included overstamps and various misprints. Your chip could be a poorly hot stamped example or could be the result of a broken die.
